On behalf of all our staff, we welcome you to Day Hill Dental. We are pleased that you have selected us to care for your dental needs. Preventative dentistry is our goal for every patient. We want you to know that we are committed to providing you with the highest quality of oral health care in the most gentle, efficient and caring manner possible.

On this site you will find our Health Questionnaire and Notice of Privacy Practices. Please complete and sign both forms and bring them with you to your first visit. If you have dental insurance, please bring the card with you. 

On your first visit with us, we will listen to your dental concerns and answer your questions thoroughly. You can expect an examination including necessary radiographs, and a discussion of the most appropriate treatment to meet your dental needs.

Dr. Bloom, Dr. Chu or Dr. Farber and staff are dedicated to providing you with a pleasant visit and results that you’re proud to show off. Our administrative staff is ready to help you with questions about scheduling, financial policy and insurance, to make that part of the process as simple as possible.
