This amount may be subject to adjustment when the dental service(s) claim(s) are reviewed by the insurance company. Insurance companies have annual maximums for the amount of dental services that can be reimbursed within each year. If you or your family exceed the annual maximum you will be responsible for the full amount of treatment that exceeds your plan’s limitation. The patient is responsible for monitoring the amount of his/her remaining benefits for any annual benefit period. You as the patient are always responsible for any charges that are not covered by insurance.

Claims we submit to the insurance companies indicate that you have assigned those benefits to Day Hill Dental. If the insurance company pays you instead of Day Hill Dental, then you become responsible for the total account balance.
